Open the APPLICATION FORM and fill it out.  You can then send the form by pressing the SUBMIT button at the end of the application.  This will give you the option of emailing the completed form through the mail service of your choice. Please attach a copy of your high school diploma or GED, and a resume to this email.


This is a three part form: the firs is the Application for Employment, the second is the Criminal Record - Self Disclosure, and the last part is the Consent to Release Information.  All parts must be completed before emailing or delivering the application.


You will need Adobe Reader to view/print/submit the PDF form.

To apply in person bring the completed Application form, a copy of your high school diploma or GED, and a resume to our Main Campus: 944 S Arizona Avenue, Yuma.  The Human Resources office is located inside the Men's Shelter building
